Ms. Vanzant has been with us a couple times. She then came to us for some additional therapy in June. You wouldn’t know it by looking at her, but Ms. Vanzant is almost 90 years old and has been happily living by herself at home for many years. After her first visit with us a fow months ago, she successfully discharged home with home health services. With her most recent hospitalization at Baylor in Grapevine, our discharge planning team helped coordinate new living arrangements at the Dancing River Assisted Living Facility. She is scheduled to discharge there soon and has been a pleasure to work with! Originally from Magnolia, AR, Ms. Vanzant managed apartment complexes and a branch office for Edward Jones before retiring. We are pleased to service patients with great outcomes!
